Professor X Drug L synergy

I was trying out mutants in aq map 6 yesterday. The synergy says once per fight if a champ were to get knocked out they would regen x% of hp back instead. It triggered in a fight against darkhawk and I moved on to domino and after a few crit fails I was low health. No unlucky debuff was on me when my hp went to 0 but the ability didn’t trigger and I lost him. I’m fairly certain this has happened to me last month in quests (i.e. it will trigger once but later down the line it won’t do it again). So is this ability supposed to be a once per quest one or should it trigger multiple times?

    You passively lose 15% AA when u start a fight against domino. The unlucky passive increases this to 50%. It is not needed for your abilities to fail.
