Scarlet Witch not creating buffs in AQ

Since the latest patch Scarlet Witch has been creating only one buff at a time or none at all during a fight in AQ. There have been multiple fights where she would only get dexterity. My last fight against dormammu (AQ) each level 2 (4) only hit for 2500 vs 15-20k typical. Durning a story quest there are times she will crit, and had an increased crit rate, but not indicate the symbol. This is only noticeable based on multiple hits with high damage...This is not occurring during AQ. My SW is 5/50 lv 76, but is performing like an unduped 4/40. Hopefully this issue can be addressed soon. Thanks.
