Caltrops? It’s an old node, a bleed immune would be great for Blade and Morningstar.

The caltrops node is the culprit, as it causes you to bleed if you dash back. So yes, you can die without getting hit if you keep on dashing back without a bleed immune.

Another boss later in the run, Morningstar, can also kill you without hitting you because she inflicts bleed on you 50% chance every time she hits you. So lesson learnt, bring a bleed immune for this boss rush, as well as an incinerate immune for Mephisto (ie Iceman)
