Hercules immortality not triggering in AQ

Anyone else having trouble with Hercules not triggering his immortality? I was fighting the second mini boss HT in AQ Map 5 and it hasn't triggered once since I've played this morning. There are no nodes that reduce ability accuracy. Or is there something else I'm missing. The first two AQ's his ability triggered. *Edit* Figured out how to post a video
