Does anyone know the exact scaling for the defensive ability accuracy reduction? It's not listed for some reason
A lot of older champions have slightly vague abilities. Take a look at Black Bolt's Sig ability, which gives him 'a chance' to proc a buff when struck.
A long time ago, one of the mods gave the impression that Gwenpool's DAA was about 2% per hit. This has largely become accepted fact; but I don't know if anyone's dug into the coding to check it out.
On the other hand, there have certainly been 'Bug' threads here and there about her opponents defensive abilities triggering at 50+ hits, and they never seem to get answered.

It might max out at a certain point that isn’t mentioned

It's been a bit, so I thought I'd re-verify it, just checked and it's still 2% per hit in her combo. It doesn't appear to be capped at 100% either, so champions with >100% ability accuracy could be reduced further if nothing else prevents it.
On the other hand, there have certainly been 'Bug' threads here and there about her opponents defensive abilities triggering at 50+ hits, and they never seem to get answered.
I might be able to explain why that would happen (I just explain, I don't justify that it's right, or intentional)
Gwenpool's defensive AAR is only active on the frame the attack lands (she has a separate anti-evade mechanism that also uses 2% per hit. Evade can be complicated too and there are some nuanced restrictions that may or may not come into play - things like node based evade can be tricky).
Since the ability accuracy is reduced on the attack connecting it only affects chance abilities that would trigger that frame, some abilities are multi-part, with (hypothetical example) an initial trigger, and that trigger cascades on the next frame as the trigger for the "multi-part" ability.
There's also just cases where abilities cannot be reduced to 0%
If you have an example where you know this has happened I can take a look at the ability that's getting through and see if I can shed any additional light on it.
