No Vault from SQ

Has anyone else had the issue where they have not gotten the vault from the side quest? I did the Havok path first by fluke, I got my shards, but no vault. I later heard that the Havok path had the vault, so I actually did a second pass, still nothing. Now I have completed all paths and I haven’t received the vault from any of them.

Edit: Didn’t see this when I Googled but have seen other threads now, please delete.
