Missing Incursion Boss Hack

While doing a Sector 6 Incursions Run, I had gotten a few rooms in before taking a break for a few days. Upon returning, one of my two boss hacks, being At The Ready, was no longer displayed under my list of hacks. Despite this, it still functioned in combat. The problem lies in the fact that upon my next boss hack selection, it came up as an option, and muddled my pool of options? Does anyone know what caused this? I have attached a YouTube video as evidence, and the video thumbnail is when I was offered At The Ready again, you can see from my number of hacks that this happened after my recording.
