Bug or I have to explore this?

It shows I have done the path but some is unexplored. Act 4. Tells me it is 91% explored. Also happened to me on an act 5 quest but I didn’t care then I just explored

Not a bug. They revamped the earlier acts and removed some paths. As a result sometimes unfinished paths were put on one of your already done paths.
