Black Cats second medium failing?

I’ve had it a lot over the last month or so, I’ll have a second special ready, I’ll use my striker, go in for a full 5x combo ending with a second medium…. But somehow in between using the medium and activating my special, the defender manages to put up a block and smiles at my attempts.
Just had it happen with a 6 star Johnny Storm…
Is it failing due to accuracy? Or is the game just managing to use that 0.01 second of space in between to pull up a block?
