Catylists not appearing in inventory!

This has consistently been a problem for me since I got the game. I'm not very good, so I've only ever received tier 1 and 2 catylists from different quests. However, after completing a quest or doing a run on proving grounds, it will say I got a catalyst however when I go to my inventory it's not there, and there's never a sign I got anything from that quest at all. The strange thing is that the only time my items don't show up is when there's a catalyst amongst them, but when there isn't all my items appear. As I said, I'm relatively new this game so I could just be missing something but any help would be appreciated.
