Mephisto bug?

I was using Mephisto for today’s vanquishing blow event in RTTL 1.2. On the hulk fight I used my heavy and it didn’t kill him and hulk’s Hulk Smash fury activated. This was less than 5 seconds into the fight. I though soul imprisonment was 100% chance to prevent enemies from activating buffs.
