Being kicked out of quest and having all progress lost

This has happened to me twice now and I'm pretty sure that I'm not crazy. I'll finish fighting someone and it will send me right back to the home screen, I wont be in an active quest or event, just be back in the home screen and when I go back to what I was doing all of my progress is gone.
Has this been occurring to anyone else?

Description of the Issue: After my battle in my Merc Mission run I get sent back to the home screen and exited out of the event/story quest.
