Defenders being placed on AW defense but not showing?

Is anyone else experiencing this?
Our alliance has had 4 different people in the last 3 weeks that swear they have placed all their defenders but when looking at the defense placement moments before war starts, only 4 out of 5 were placed. We have managed to get them all placed again properly at the last minute except for one time. We went to war with 4/5 placed. Weird, because it shows the 5th defender on the list with no node.
It seems too much of a coincidence that 4 different members say they placed all 5 of their defenders.
