Tigra Heavy Miss Fails

Just got blasted twice by the projectile part of Apoc's special 1 in Uncollected EQ 2.2 while Tugra was charging her heavy. Recently she's been getting hit by certain Champs while charging a heavy when she shouldn't. Noticed especially that it is hit and miss with the second have of Ebony Maw's special 1, as well.
I first noticed it happening in Battlegrounds, but has also happened in AW. Last night it cost me a death in AW. I thought it might be the assassin mastery, but I wasn't below 18% health and since it is happening in EQ, no assassin present or AAR of any kind.

    At our tier in AW the boss has true strike, but not true sense, so not that either.

    Neither of those would cause her heavy miss mechanic to fail.

    Weakness, Slow and Exhaustion deactivates her ability to miss projectiles. Apocalypse has Weakness on his SP1 that he places automatically on you if you're suffering from a debuff. Since he also stands on a node that places small incinerates on you if you're not super aggressive, he most likely shut down your ability to miss as soon as he activated his SP1.

    As for Ebony Maw, I don't think any of the hits in his SP1 count as a projectile.

    All of Ebony Maws sp1 is Contact/Energy attacks.

    I've used Tigra against EM and Apoc's L1 many times without incident.
