Black widow boss not setting traps at all ( not referring to invisible traps here)

I was doing the tech path, and within the fight BW stopped setting traps like at all not invisible traps she's not setting any traps while she's at 10% as shown under her par of power no number 1 or 2 is showing , and of course not taking any damage, am I missing something here or what's exactly happening

    This happened to me too. I got her down to around 11% with Colossus and then brought in Kitty. No 1 or 2 showing and got the indicator that it was the final phase even though it wasn't 6% or under yet. I had to restart the game three times before it finally came back. I had taken the Adrenaline Rush/Mutant path.

    Same, had to restart a couple of times which costed me a few extra revives, seems like another bug here
