It happens. I know various alliances have had an issue. Someone misses a role node but everything still comes down and the whole BG loses exploration for a role node. If everything comes down why cost exploration over a missed role node? It’s not a fight.

There wasn’t even 9 people in Raids (and at an extreme, only 3 people), but they defeated all the defenders. Should 3 people (all on different sections) really be able to qualify for “Explore”. It is actually a job well done, and maybe deserving if only with 3 people, but no.
People joining in light participation, so thinking there won’t eventually be enough people to cover all paths, so might as well make sure those that do join can at least get all the Chests from a single path. And then later, enough other people do join, and then it is “why didn’t we cover 9 paths ?”
Or similar to above, but expecting that with 10 people there will be one “double up”. And so someone earlier decides to be that “double up” person, well before making sure all other paths eventually got covered.

Lastly, yes, occasionally, someone will not be very observant, and joins with either the incorrect Role in which to cover the last remaining path. Or also unobservant and takes a wrong section (even though they had the correct Role).
Actually, very lastly, 2 people join at basically the same time (with same Roles, unable to see that another person is also in the process of joining too), and so potentially end up stuck with too many of a given Role. (In theory, 10th person should be able to make up for that).
**which could warrant 1 extra change beyond what they recently did locking in a person to a path based on Role they brought. That extra change could be “Allow a Champ Swap” right at the beginning. In case people join at the same time. (Or just wasn’t paying attention to Roles/Paths before joining.
So AQ raids was added to fill in that gap. As normal AQ lost all those elements, raids gained them all and then some. So I believe the notion that a "simple mistake" could cost exploration rewards is intentional, because the point to Raids existing at all is to reward alliances that actually do all that stuff correctly every time.
