I just need clarity on whether or not you have to %100 explore ALL difficulties, or %100 explore A difficulty level?

The stone doesn't show up in inventory or anything. It shows up connected to the associated champion. If you've collected the Soul Stone, you'll see it when you see OG Cap in your roster or when you try to enter the Save the Battlerealm event.
