Did any of your start and then quit out of the quest? That would cause this to occur, and is intentional, but if you completed it without quitting and had this issue, please let us know.

To stop people from gaining more attempts. The system can't determine the difference between human error and force closing.

If it was a difficult quest that can only be completed with great skill or a strong roster I could understand limiting the attempts. Those quests are meant for a select few players, so giving lesser players unlimited tries to get lucky would be contrary to the purpose of the quest.
This quest, on the other hand, is trivially easy and meant to be completed by everyone, so the only possible purpose of limiting attempts is to punish people for making a silly mistake.
