Do you have to complete all the difficulties to move on to the next or is this a bug?

    As to the different difficulties. NO, those are different objectives.

    As to each objective, last 2 replies are correct. And they do have to be completed in order (week 1 quest with Mystic, then Mutant, then week 2 quest with Cosmic, etc…), just pay attention to *which* quest you are trying to use those teams in.
