1-Hour Hero (and Featured in general) Pic shows 2 heroes, but sometimes only 1 is Feature ??

Why is it that some of the Special 1-hour Feature Hero Crystals show 2 heroes on the graphic, when (especially these 1-hour versions) it is clearly only 1 hero that is the extra odds Feature ??
And does that mean that Featured crystals in general we should be looking at the exact WORDING on it to know whether both or just one from the graphic would have extra odds ? Because plenty of the monthly Featured show 2 heroes on graphic , but only about half of them mention the 2nd hero in the Description.
Please Clarify for us (especially regarding normal Monthly Featured), it is always confusing to know exactly which have improved odds. Thanks.

In this case (as a 1 hour feature) we know it is only Medussa. But what about when this crystal was out before (or one like it in the future), would Kingpin have been a secondary increased-odds hero ? And if not, then why is he on the graphic.
