I had an issue fighting a Mephisto in Variant #3 Chapter 2 Quest 2 with Quake's concussion not working. I was able to evade an attack with the way her concussion works even though it didn't show but the effects of her concussion did not work so he was still able to heal.

iPhone 6s here and I am having the same issue. I thought I was going to go and need to get a battery replacement but looks like it's something with the new MCOC update.

It is 5%. You can look this guy up in-game, roastedbagel, and see his 6star Killmonger. You can check the synergies and see that it is still only 5%.
